For AAAs, case managers, home care and health plans

You can arrange the meals, the rides, the aide. You can't schedule a friend.

Friend Loom is a private online community for older adults: a hosted small group that meets weekly, plus member-led events all month long. Refer a client in two minutes and they gain people who know their name, without leaving the house they want to stay in.

Case manager sitting with an older client at her kitchen table

“It's the one referral I make where the client calls to thank me.”

Social isolation is the gap in every care plan.

You screen for it. You document it. Then you look for somewhere to send them, and the options are a congregate site they cannot get to, a volunteer caller who rotates out, or nothing.

Aging in place, alone in placeStaying home is the goal. Staying home by yourself for six days out of seven is the side effect.

Transport ends most programsIf it requires a van, a ramp, a good weather day or a good pain day, attendance falls off in a month.

A friendly caller is not a friendshipOne volunteer talking at a client is kind. Belonging to a group of peers who expect them is different.

Isolation lands back on youThe lonely client calls the office more, cancels less carefully, declines faster, and shows up in the ER on a Sunday.

Why partners refer

Eight reasons this fits a real caseload.

01 Works for homebound clients

No transport, no weather, no stairs. If they can answer a video call on a tablet or phone, they can attend everything.

02 Community, not a check-in call

They join a group of peers, share their story, follow the members they click with, and dial into events all month. Connection that keeps going without you.

03 It actually sticks

Same hour, same faces, a trained host, and a real person who notices absence. Habit is the whole design, so referrals do not quietly lapse.

04 Two minutes of your time

Send a name and an email or phone number with the client's permission, or hand them a printed card and let them self-enroll. No portal to learn.

05 No protected health information

We need contact details and interests, never a diagnosis, chart or assessment. Nothing about the referral is published or sold, and members have no public profile.

06 Safe by design

Every member is seated by a human, every room has a trained host, nothing is recorded, and no one can be solicited for money inside the community.

07 A visible answer to a documented need

Close the loop on the social isolation item in your assessment with a named program, a start date and a standing weekly time.

08 Relief for the family too

Adult children stop carrying the entire social load of a parent, which shows up as calmer, less anxious conversations with your staff.

Who this is built for

If isolated older adults cross your desk, we can take that handoff.

Area Agencies on Aging & ADRCs

A referral for the caller who is not eligible for much else, or is waiting on a service list.

Case managers & social workers

Something concrete for the social isolation line in the care plan, with a date attached.

Home care & home health agencies

Fill the long hours between visits, and give aides a client who is in better spirits.

Health plans & care management

A supplemental benefit or community referral that addresses loneliness for members at home.

Senior centers & libraries

Keep serving the regulars who stopped coming in, and the ones who never could.

Faith communities & hospices

Companionship for widowed members and bereaved spouses, without another building to reach.

Referring takes about two minutes.

Three steps, and the last one is not yours.

1

Ask, then send

With the client's consent, send us their name, contact details, time zone and a line about what they like talking about.

2

We seat them

A person places them in the right group and sends their link with a big-print guide to dialling in. A practice room stays open for a dry run.

3

They belong to something

Their weekly group, the events they choose, the members they follow. You get a confirmation that they were seated and started.

The community

Not a call you attend. A community you belong to.

Friend Loom is a private online community for people who want real connection. You share your stories, you follow the members whose lives interest you, and you join as many events as you like from your computer, phone or tablet. Members lead most of them, so the calendar changes every month. Nobody phones you, and nobody minds if you sit a week out.

Your people

Follow the members you click with

Every member has a page inside the club: where they are from, what they know about, the stories they have told. Follow anyone and you will see when they post or host something. Private to the community, invisible to the open internet.

Sharing

Tell your story properly

Post the long version: where you grew up, the job nobody knows about, the year everything changed. Members read it, ask about it, and remember it next time you speak.

Standing groups

Circles for the things you love

Smaller rooms built around one thing: books, cards, walking, genealogy, veterans, caregivers. Join as many as you want, or start the one you wish existed.

Member led

Anyone can host an event

Put your own event on the community calendar: a hymn sing, a trivia night, a walk and talk, a lesson in the thing you are best at. We help you run it and fill the room.

Everyday

A quiet room, always on

Photos, recipes, the finished puzzle, a thinking of you. Conversation carries on between events, with no feed to scroll and no likes to chase.

Somebody notices

You are missed when you are gone

Birthdays get sung to and someone notices when you miss the events week after week.

Events all month long, most of them member led.

Join from your computer, phone or tablet, or dial in by phone.

A regular rhythm every week, plus whatever members put on the calendar this month. Come to one, come to six.

Your loom hour · weekly

The same 4 to 8 people at the same hour, hosted, so it never turns into an awkward silence.

Life story hour · weekly

One member takes the floor and tells their story. Everyone else listens and asks good questions.

One thing I learned · weekly

A member teaches the community something useful: sourdough, birding by ear, reading a Medicare letter.

Joke jar · weekly, 20 minutes

Everybody brings one joke. Groaners are not only allowed, they tend to win.

Open rooms · most mornings

Coffee hours with no agenda. Stay five minutes or fifty, camera off if you like.

Member events · all month

Trivia, hymn sings, garden tours, film clubs, birthday parties. Whatever the community decides to run.

What partners ask before the first referral

Does it cost my organization anything?

No. Referring is free, and there is no contract to sign. Membership is a subscription the member or their family holds. Current pricing →

What if a client cannot afford it?

Tell us. We keep a small number of sponsored seats for partner referrals, and family members often cover it once they understand what it is. Ask about block sponsorship if your organization wants to fund seats directly.

My client is not good with technology.

Most are not, at first. Joining is one link and one large button, we send a big-print guide, the practice room is always open, and a human answers the help line. Members who have never used video before are regulars within a few weeks.

Is this a clinical or crisis service?

No. Friend Loom is company, not care. Hosts are trained conversation keepers, not clinicians, and we escalate to the contact on file or emergency services if someone appears to be in danger. It complements your services, it does not replace them.

Will I hear how the referral went?

You get confirmation that they were seated and attended their first weeks. Beyond that their community is private, and what they say in a room stays in the room. See our privacy policy and security & compliance.

Can we refer at volume?

Yes. We open new rooms as groups form, and we can hold a room for a cohort from one county, plan or agency so your clients are seated together.

Start with one client you worry about.

Refer them this week, and see what a standing hour and a community does by the end of the month.
